We collect health information about children, which is among the most sensitive information there is. This policy explains what we collect, why, who can see it and how you can access it. It applies to our website and to our clinical services.
Who We Are
Collaborative Partnerships Pty Ltd, a paediatric allied health practice at 4/9 Goshawk Boulevard, Buderim QLD 4556. We are bound by the Australian Privacy Principles under the Privacy Act 1988 (Cth) and by Queensland health records legislation.
What We Collect
Contact and identity details for the child and their parents or carers; health, developmental and educational information relevant to the care we provide; assessment results, clinical notes and reports; funding and billing information including NDIS or Medicare details; and correspondence with you and, with your consent, with other professionals involved in your child's care.
How We Collect It
Directly from you, through our enquiry form, by phone, by email and in appointments. With your consent we may also receive information from your GP, paediatrician, school, kindy or other treating professionals. Our website enquiry form is provided through a third-party platform and submissions are stored in our client management system.
Why We Collect It
To provide safe, effective clinical care; to write assessment reports and progress reports for you and, with your consent, for referrers and funders; to administer bookings, billing and funding claims; to meet our professional, legal and insurance obligations; and to contact you about your child's care.
Who We Disclose It To
Only where you have consented, or where the law requires or permits it. That typically means other professionals involved in your child's care, your funder where a report or invoice is required, and our software providers who store the information on our behalf under contract. We do not use your child's information for marketing.
How We Store and Protect It
Clinical records are held in access-controlled electronic systems, and staff access is limited to what their role requires. We retain children's health records for the period required by Australian law and our professional obligations.
Website Analytics and Advertising
Our website uses Google tools to understand how the site is used and to measure the effectiveness of our advertising. These set cookies in your browser. You can block or delete cookies in your browser settings without losing access to the content of the site.
Accessing or Correcting Information
You can ask for access to the personal information we hold about you or your child, and ask us to correct it if it is wrong. Write to us at admin@collaborativepartnerships.com.au. We will respond within a reasonable period, and will explain our reasons if we are not able to give access.
